Step 1: From Hospital to Home Care

Our team at Care Plus meet with you and your family members, and the medical team to establish a personalized care plan.  As a leading Home Care agency, we put together a guide to provide an overview of the challenges and resources associated with each step in the transition from hospital to home. The guide will first describe what the discharge process entails and the key players involved.  The guide will also provide a step-by-step summary on ensuring optimal care post-hospitalization. 

Step 2: Caregiver and Training

Our caregivers undergo training in the facility and/or externally and with the rehab team (PT/OT) to ensure full compliance with care plan specifics.

Step 3: Around-the-Clock Care

Care includes assistance with personal care, lifestyle support, light housekeeping, errands, and more!
