Care Plus offers a comprehensive nursing and home health care program. The rate schedule listed below includes regular visits by a Registered Nurse who will monitor the patient’s condition, supervise our staff and speak to the physician as required. Patient services are conveniently scheduled by our office and personnel attending to the client's care are paid biweekly and are covered by Malpractice and Liability Insurance, Worker's Compensation and for all benefits required by law.

NOTES: All rates are time and one half of services if rendered on Holidays. A visit is expected to last two (2) hours or less. Hourly rates are based on four (4) hours minimum. Visits more than (2) hours and less than (4) will be billed at (4) hour minimum.
INSURANCE COVERAGES
Care Plus will be pleased to assist you with your claim by providing registration numbers, etc. With your permission, many Insurance Companies will allow us to bill them directly, thus relieving you of this procedure. We also recommend you be completely familiar with your policy, coverage and limits, because clients are responsible for all deductibles, user fees or charges not covered under their policy.
