For Full Time Employees (36+ hours per week):
Health Care/Dental-Care Plus Provides a comprehensive Health Care Package through Medica. The plan has a reasonable deductible per year and a small co-pay for office visits, generic drugs have a smaller co-pay and name brand drugs are a bit more. Care Plus pays half of the premiums for your personal coverage. You can add your spouse and children to the plan as well. The premium is automatically deducted from your paycheck every two weeks once you are enrolled, and the money is taken as a pre-taxed dollar. To qualify you must work at least 36 hours per week for 90 days. The dental plan is offered through Delta Dental and covers at different percentages depending on type of work performed. You receive 2 free cleanings per year, and premiums are deducted pre-tax as well.
Life Insurance-Care Plus provides a $10,000 Death Benefit for you free of charge. Ask for your free insurance application when you become eligible for Health Insurance through Care Plus H.H.A. Inc.
PTO-Paid time off is based on quarterly pro-rated hours and can be used for vacation, sick, or personal days.
For Full and Part-time Employees:
Retirement Plan-Care Plus offers a SIMPLE IRA plan. You become eligible after 1 year of employment and must earn at least $5000 during that year and every year thereafter. Once you are eligible you have the option of deferring a portion of your gross pay tax free, and if you are 50 or older you can defer even more! If you elect to contribute to the IRA the deductions will be made automatically from each paycheck you receive. You may elect to change the percentage of your contribution twice per year in January and July and as an added incentive Care Plus will match up to 3% of your contributions!
Mileage Reimbursement-Care Plus pays your mileage between cases at a rate of $.48/mile and also offers discounted bus passes.
Aflac - Care Plus offers all Aflac benefits for employess that have worked a minimum of 90 days.
